Request for Access to Hospital Medical Record Information

Victorians have the right to access or correct health information our organisation holds about them.

Access to health information requires proof of identity. Requests made on behalf of another person must be made in writing, and must include evidence of their authority to act on behalf of the other person.

Should you wish to make an application to access information from your medical record please complete the application form and post it to Ramsay Clinic Albert Road together with a photocopy of photo identification (e.g. your driver’s licence or passport) to:
